How does filling in the form work?

It's addressed to you and shows your venue's name at the top.
Answer the questions
Prefer Français or Español? Switch language at the top — the choice is remembered for your next form. If the venue allows drafts, you can save partway and finish later on the same device.
Sign where asked
Draw with your finger or type your name (a typed name is rendered as your signature). Required signatures must be completed before you can submit.
Submit
You see a confirmation, and your answers go to your venue — nowhere else.
What if I'm re-signing an updated form?
Your previous answers are shown, but only the signature and date fields are open — the rest is locked to what you already submitted.
Common questions
The link says expired — what do I do?
Form links expire on purpose. Ask the venue for a fresh one — their contact details are on the expired page.
I made a mistake after submitting — can I edit it?
No. A signed form can't be edited afterwards. Contact the venue and they'll send a new form to correct it.
Who can see my answers?
Only your venue. Answers travel over the private link straight to them.
