Review a submitted form

Open a client's form submission to read answers and signatures, fix unsigned responses, and see why signed answers are locked from editing.

Open Clients → Submissions and click a submission to see everything the client entered — answers, signatures, uploaded files, delivery status, and internal notes. Unsigned answers can be corrected; signed ones are locked and must be re-collected.

What's on the submission detail page?

The header shows the client, the template, and the submission's status:

Below it:

  • Response data — every answer, in the order the form asked. File-upload answers render as real download links.
  • Signatures — each signature or initial field shows the captured signature and when it was signed, or Not signed.
  • Email status — whether the form email was delivered and opened, and when the link expires.
  • Admin notes — a free-text notes box for staff, invisible to the client.

Use j/k or the arrow keys to step to the previous or next submission without going back to the list.

The browser tab title deliberately shows a short reference instead of the client's name, so a screen-share or a glance at your tabs doesn't leak who submitted what.

Chase an unfinished form

For a submission still in sent, viewed, or expired:

  • Resend emails the client their form link again.
  • Copy link copies the fill-in link so you can send it yourself, or open it for a client at the desk.

Edit an answer — and when you can't

If your role can manage submissions, completed responses can be edited — the pre-signature typo-fix path. Every edit is kept in an edit history.

Signed responses are locked. Once a client has signed, editing any answer is refused — a consent with silently changed answers under a signature would be worthless. To correct a signed submission, re-send the form and collect a corrected, re-signed response; the original stays intact as the record of what was actually signed.

Don't work around the lock by keeping corrections in the notes box for anything clinically or legally material — re-send the form. The signature must cover the answers.

Masked answers (PHI)

If a form is marked as containing protected health information and your role doesn't include clinical chart access, the answers appear masked. You can reveal them for that one submission when your work requires it — the reveal is recorded in the audit trail with your name. Accountability, not a hard wall.

Download PDF produces the completed form — answers and signatures — as a PDF for printing or external records.

Common questions

Use Resend on the submission — it emails them a fresh link to the same form. Or Copy link and send it however you like.

Can I change an answer after the client signed?

No. Signed responses are locked; re-send the form and collect a corrected, re-signed response. The original stays on record.

Why are the answers hidden behind a mask?

The form contains protected health information and your role doesn't include chart access. Reveal shows them for that submission and records the reveal in the audit trail.

Who can edit unsigned answers?

Anyone whose role can manage submissions — it's a capability, not a job title. Edits are kept in the response's edit history.
