Send forms and consents

Build a form once, send it to one client or many, and track it from sent to signed.

Forms cover intake questionnaires, medical history, consent, and post-treatment sign-off. You build a template once and send it as many times as you need.

Build a form

Clients → Forms → New form

Add fields, group them into sections, and mark the ones that are required. Start from a blank form or one of the starter templates.

Per-form settings worth knowing:

  • Category — how the form is grouped in the list and in a client's documents
  • Signature — whether a signature is captured at the end
  • Expiry — a default number of days a sent link stays live

Each template has version history, so you can see what changed and when. A form already sent to a client keeps the version they were sent — editing a template never rewrites a completed submission.

Archive a form you no longer send instead of deleting it. Deleting orphans the submissions that reference it.

Send a form

Clients → Submissions → Send, or from a client's profile, or from the Send button in the top bar.

Pick the form

The active templates you're allowed to send.

Pick who gets it

One client, several selected from the directory, or everyone carrying a particular tag. If the person isn't in the directory yet you can add them inline without leaving the send screen.

Set the expiry

Default seven days, adjustable. The screen tells you the exact date the link will stop working.

Choose the email

An email notification goes with the form by default, using one of your email templates. You can turn it off and share the link some other way.

Send

Each recipient gets their own private link. Nobody can see anybody else's answers, and the link carries no login.

Track a submission

Clients → Submissions lists everything sent. Each one moves through these states:

The list flags forms that were sent more than 72 hours ago and never opened, so a stalled intake gets chased instead of forgotten.

You can resend, extend, or cancel outstanding forms in bulk from the list.

Attach forms to a service

A service can require a form. Set it on the service under its Forms tab, and the form is offered — or required — as part of that booking.

If a service requires a signed clinical form and it isn't signed, checkout is blocked for that sale. The message names the form. Send and complete it, then settle.

Filling a form in the clinic

Kiosk turns a tablet at the front desk into a self-serve station: the client picks their form, fills it in, signs, and hands the tablet back. The kiosk is PIN-protected so a client can't wander into the rest of the app, and it locks out after repeated wrong PINs.

Reading the answers

Completed submissions appear on the client's Documents tab and in the submission list. Open one to read the answers, see the signature, and download a PDF.

Health information in forms is treated as sensitive. Access to submissions and clinical charts is permission-gated separately from the rest of the client record — see Roles and permissions.

Reporting

Growth → Reports → Forms funnel shows how many forms you send, how many are opened, and how many come back completed.
